Documents To Submit With The PPP Forgiveness Application

Additional Documentation May be Requested

Verification of FTE

  • Federal tax forms (filed or prepared and not yet filed), typically IRS Form 941 - State quarterly business and individual wage reporting and unemployment insurance filings from January 1, 2020 and the end of covered period. Not required to be submitted with 3508S applications.

Cash Compensation

  • Third–party payroll reports or
  • Bank Statements highlighting the amount of cash compensation paid to employees

Health Insurance

  • Proof of payment from covered period. This can be cancelled checks, payment receipts or account statements.

Employee Retirement

  • Proof of payment from covered period. This can be cancelled checks, payment receipts or account statements.

State and Local Taxes*

  • Payroll Report, State quarterly business and individual wage reporting and reported unemployment insurance tax filings.

*Only State Unemployment (SUI) and Employment Training Tax (ETT)

Owner Compensation

  • Proof of 2020 compensation through covered period - Third-party payroll service provider reports 2019 and
  • Tax forms/schedules, including:

-2019 (or 2020 if loan disbursed in 2021) Form 1040 Schedule C for self-employed/sole proprietor/Independent Contractor (Non-Farmer)
-2019 (or 2020 if loan disbursed in 2021) Form 1040 Schedule F for self-employed/sole proprietor/independent contractor (Farmer)
-2019  (or 2020 if loan disbursed in 2021) Form 1065 including K-1s (partnerships).

Business Mortgage Interest Payments*

  • Documentation must provide evidence of existing mortgage obligation prior to 2/15/20—Lender amortization schedule or February 2020 account statements and
  • Copies of invoice and proof of payment from covered period. This can be cancelled checks, payment receipts or account statements.
  • *Only the interest portion of mortgage payments are an eligible expense.

Business Rent or Lease Payments

  • Documentation must provide evidence of existing lease or rent obligation prior to 2/15/20—Copy of lease or rental agreement and
  • Copies of invoice and proof of payment from stated covered period. This can be cancelled checks, payment receipts or account statements.

Utilities (Electricity, gas, water, transportation, and internet access)

  • Documentation must provide evidence of existing obligation prior to 2/15/20 and
  • Copies of invoice and proof of payment from covered period. This can be cancelled checks, payment receipts or account statements.

Covered operations expenditures

  • Copy of invoices, orders, or purchase orders paid during the Covered Period and receipts, cancelled checks, or account statements verifying those eligible payments.

Covered property damage costs

  • Copy of invoices, orders, or purchase orders paid during the Covered Period and receipts, cancelled checks, or account statements verifying those eligible payments, and documentation that the costs were related to property damage and vandalism or looting due to public disturbances that occurred during 2020 and such costs were not covered by insurance or other compensation.

Covered supplier costs

  • Copy of contracts, orders, or purchase orders in effect at any time before the Covered Period (except for perishable goods), copy of invoices, orders, or purchase orders paid during the Covered Period and receipts, cancelled checks, or account statements verifying those eligible payments.

Covered worker protection expenditures

  • Copy of invoices, orders, or purchase orders paid during the Covered Period and receipts, cancelled checks, or account statements verifying those eligible payments, and documentation that the expenditures were used by the Borrower to comply with applicable COVID-19 guidance during the Covered Period.

In addition to the documentation listed above, each borrower is required to retain (but not deliver) a number of supporting documents as part of the loan forgiveness process for six years. Please refer to page 4 of SBA Form 3508EZ Instructions, page 6-7 of SBA Form 3508, and page 3 of SBA Form 3508S Instructions for a listing of such documents.
